If you have purchased PC Survey and need a permanent authorization, please fill in all fields so we can make sure to give you the correct authorization.
If you only want a temporary 30-day authorization, then only your e-mail address, Site Code and the date of the program are required.
We will process your authorization request as soon as possible. |
| Note: We can not give you an authorization without the Site Code. The date of the program is also critical. The Site Code can be found in the Project menu under Authorize. If the date is not also listed there, it can be found under Project --> About. If you cannot open the program to where it says "PC Survey is not authorized," then filling out this form will not help.
